Louisiana Health Cooperative, Inc. in Rehabilitation (LAHC) is operating under a Permanent Order of Rehabilitation entered on September 21, 2015 by the 19th Judicial District Court, Baton Rouge, Louisiana. The Louisiana Insurance Commissioner was appointed by the Court as Rehabilitator and Billy Bostick as "Receiver" of LAHC. All LAHC policies terminated on December 31, 2015 by order of the court.
LEGAL NOTICE is hereby given that any and all claims against LAHC MUST BE FILED BY MARCH 31, 2016, unless for good cause shown and at the discretion of the LAHC Receiver.
ANY CLAIM RECEIVED AFTER MARCH 31, 2016 will be considered untimely and not currently eligible for payment.
All required premiums, deductibles, co-payments and co-insurance payments must be paid in full. If your coverage terminates, you will be fully responsible for the cost of health services that you received after the termination date.
Providers should continue to submit claims in the ordinary course of business. Any duplicate provider claims will be charged $10.00 a line for processing.

On December 14, 2015, the 19th Judicial District Court for the Parish of East Baton Rouge, State of Louisiana has entered an order approving the LAHC Plan to continue coverage for LAHC policyholders, members, subscribers and enrollees through December 31, 2015 and partial payment of claims in order to avoid the immediate liquidation of LAHC leaving thousands of Louisiana citizens without healthcare.

Louisiana Health Cooperative was placed in Rehabilitation by the 19th Judicial District Court on September 1, 2015 and the order of rehabilitation was made permanent by the Court on September 21, 2015.
Provider claims are now being paid in a timely manner. At this time we continue to believe that LAHC, wit the support of CMS, will have the ability to continue to pay claims. The Rehabilitation Order states that the Commissioner is entitled to enforce, for the benefit of policyholders, contract performance by any party that the contracted with LAHC. Providers are required by their contracts to continue treating policyholders.

Effective July 10, 2015, the old payer ID will not work.
Clean claims, submitted electronically result in faster payment.
LAHC members are receiving NEW ID CARDS, which have been updated to reflect the new claims submission information shown below.
